Families can prepare for a General Genetics Clinic visit by printing and completing the New Patient Intake Form or Follow Up Patient Intake Form. The completed forms can be provided to clinical staff during the rooming process, who will deliver them to the genetics team you are scheduled with that day.
Individuals seen as part of the Mitochondrial Disorders Program will need their medical records faxed to the department in advance of scheduling their appointment. View a complete list of requested records and preferred method of delivery.
